COVID RULES AND ILLNESS POLICY

The school follows the latest guidance by the CDC which no longer recommends that people exposed to COVID-19 quarantine at home, as long as they are asymptomatic and test negative.

Furthermore, although masks have and will continue to remain an important tool in preventing symptomatic and asymptomatic spread of COVID-19 and other respiratory viruses, they are no longer required. Everyone must respect the choice of others who wish to wear a mask or respirator even when not required.

COVID-19 vaccination is recommended, but not required to attend Ukrainian School.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and on behalf of myself, my spouse, my child(ren) I voluntarily assume the risk that my child(ren) and I, and any member of my family may be exposed to or infected by COVID-19 by attending and participating in Ukrainian School and that such exposure may result in personal injury, illness, permanent disability, and death. I understand that the risk of exposure may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Ukrainian School instructors, counselors, staff, volunteers, officers, directors, employees and agents.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense of any kind, that I or my child(ren)may experience or incur in connection with my child(ren)’s attendance and participation in Ukrainian School (“Claims”). On my behalf, and on behalf of my child(ren),I hereby release, covenant not to sue, discharge, and hold harmless Ukrainian School employees, agents and representatives of and from the Claims, including liabilities, claims, actions, damages, costs and expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Ukrainian School, its employees, agents and representatives, whether a COVID-19 infection occurs before, during, or after participation in Ukrainian School programming.
